Please note that this form is only to be used to make a dinner reservation at the ISCAID Gala Dinner. This form will not register you for the conference and you must use a separate registration form, if you've not already registered for the symposium. When the restaurant has reached maximum capacity, we will not be able to reserve a seat at the gala dinner for you and your name will be added to a wait list; if a seat does become available, it will be filled on a first-come basis from the wait list.

Join us for an evening of camaraderie and fellowship with your friends and colleagues at one of Dublin's premier steak houses in Centre City. Due to limited seating, only ISCAID delegates may make a reservation. We expect that we will sell out quickly.

Use this form if you want to sign up for the gala dinner separately from your conference registration. The cost to attend is $40.00 USD. If you signed up for the gala dinner on the registration form, there is no need to sign up again. When we reach the maximum capacity at Fire Steakhouse, we will not be able to take any additional reservations. Each person must sign up individually. Unfortunately, we are unable to refund your dinner reservation fees if you cancel after July 14 or are a "no show" on the evening of September 15.
